This Questionnaire is for the benefits of placing a Puppy with an appropriate family and forever home. Please answer honestly and if you have any questions you`d rather discuss over the phone, you may call me at 604 539-2100.  As I have taken the responsibility to breed these Puppies, it is my responsibility to place them in the best of homes accordingly.  This Questionnaire may be overwhelming for some but these will be questions I`d like to ask over the time of  conversing with people interested in a Puppy.  If you are living in the U.S.A. please read through the CDC Site regarding bringing a puppy into your Country. 

  I encourage intended Puppy Owners to be, to meet with me personally as I do not like to ship out my Puppies without knowing whom they are going to first.  I hope you can understand the best of my intentions.

I strongly urge a Breeder/Owners openness of communication.  This is in regards to keeping the quality of my Breeding Program up to date.  Are you willing to keep in contact with me during the Puppies lifetime and do you understand the importance of this request?

Once you have established a good bond and connection with your Puppy, I encourage all my Puppies to receive a CGN Certificate with Canadian Kennel Club or CGC Certificate with the American Kennel Club.

Home Life  
What member of the family will be taking the major responsibility raising the Puppy? 
Do you have children or grandchildren?       How many and what are the ages?  
Do you rent?         If so, does your lease allow dogs?    
Do you know if there is a height or weight restriction?

How long have you been at your current address?

Will you be willing for me to visit your home before completion of purchase of Puppy?  

Do you have a secured fenced yard and is it secured from children freely opening it?  

Can a dog dig under it or access something to climb over the fence?

Will your Puppy be in the fenced area or allowed to run loose?

Are you aware of outside distractions from your yard that may cause puppy to be bothered?

Please  let me know if you are not able to find anyone as I enjoy seeing the puppies from time to time and would love to have them come stay for Holidays and play with their Poodle Family!  I also groom if you do not have an established groomer.

Will you be willing to sign a Puppy Contract with me?      

Upon approval and agreement between myself and the new puppy owners to be, with everyone in question eager and acceptable for a new puppy to enter their lives, I will sometimes accept a reservation fee of $500.00 (non-refundable) towards a puppy.  I reserve the right to not place a puppy and return the deposit.
All Show and Breeding potentials will be sold on a Co-Ownership Contract Agreement and all Companion Puppies sold on a Non-Breeding Contract Agreement.
